Once we have your completed form, we will contact your patient to arrange a convenient time for them. We will also write you a report after completion.
We accept referrals from all dental practices both NHS and Private in Guildford and Surrey for: Cosmetic dentistry (smile makeover, full porcelain crowns and veneers), Endodontics, Advanced Periodontics (deep scale and root planing and other surgical periodontal procedures such as Pocket reduction surgery, Crown lengthening, Gum contouring, gummy smile correction, soft tissue grafting for gingival recession) , Hygienist and Air Polish, Oral Surgery (Impacted wisdom teeth extractions, third molar extractions and other complex extractions, Apicectomy, TMJ problems), Sedation Dentistry (for Children and Adults, including Inhalation Sedation and IV sedation), Dental Implants (Including full arch implants, All on 4, All on 6 and implant-retained dentures), Sinus Lifts (crestal and lateral) and CBCT Scans, OPGs, Orthodontic (invisible braces, Six Month Smiles, both fixed and removable). We also accept referrals from Hampshire and Greater London.
